form errors

Hi, I had a few folks contact me this morning that they got "form not available" error messages after submitting responses to a form I have currently open.  I don't see their attempts to submit the forms even unsuccessfuly. They tried several times.  Can you help me trouble shoot the issue?  Thank you!
