I cannot get an email sent to the recipient, no matter what I try

I have created a feedback form that is available from a CLICK HERE link on the website home page.  The form works fine and I receive a copy of the email that contains the form data but it will not send a copy of the acknowledgement email to the person that filled in the form.

The issue seems to be fixed, upon your popup form submission the form autoresponder email was successfully delivered to my email address.

Please check a Spam/Junk folder to see if some autoresponders ended up there.
