My question centers around creating a form that would allow the user to specify how many blank spaces they would have in a response. Specifically, if you ask a user how many types of titles they want (and there is no limit to the amount they could have) could the form ask a question where the user could press a "+" or something along those lines to create different response boxes?

We have those widgets which you can add in your form. They are dynamic fields in which users can add fields as many as they pleased.
