2. I was hoping that upon form submission, the integration will create a new Contact in Insightly AND would retain all details gathered from the form as a document and stored in the Contact's files. [pictured below]
-For instance, when I submitted the form I received an email with all the fields/ information that the user had filled out [pictured below]. I'm aiming for that same view of information saved as a document and saved in the Contact's files. [pictured above]

Do note that the fields mapped under the "Match your fields" in the JotForm integration wizard are the ones that will be sent to the Insightly list and that's the data that will be showing up under the details section.
If I understood correctly, you want to have an attached file with all the submitted data, just like the data you receive via email? But that needs to be sent as a file to the Insightly account.
That's not currently possible; however, please let us know if this is what you are looking for so we can open a feature request.

We have attached a feature request ticket about the Insightly uploads to this thread and forwarded it to our developers. There is no ETA for when it will be implemented, but we will let you know if we have any updates.
