Why is my account suspended?

My account has been suspended and form has been disabled. I made a form to collect details of teachers working in my classes.

I have reviewed the account registered with the email you provided and I can see it has been auto-suspended due to the form http://www.jotform.me/form/71926078627466, seems like it was suspended due to the fields asking for bank information:
