Is this suitable for medical centres and or creating "employment medical assessments"?

Also we are wanting to do more of our jobs without paper, so possibly upgrading and getting tablets for our practices and having all forms on the apt and patients to fill these out online.

    Welcome to JotForm.

First of all, let me suggest you sign up a free JotForm account and experience JotForm at the following URL: www.jotform.com/signup

Please check the following guide in order to learn how to create your first web form: https://www.jotform.com/help/2-How-to-create-your-first-web-form

You are able to create every kind of forms in JotForm, including medical forms. We have some example templates as you can see one of them at the following URL: https://support.jotform.com/form-templates/medical-history-form

You are able to collect patient information, create employment medical assessments, set doctor-patient appointments and do some calculations on a form.

Also, the forms are mobile responsive. So, there will not be any problem with fulfilling and submitting forms on tablets and mobile devices, besides desktops. You are also able to print the forms as hardcopy.
