How does Form Views limits work

One thing which is not clear to me is the 1000 views limitation. Do you count how many times the page was loaded in the web browser? Is that per IP or regardless? When does the counting reset? 1st of each month or from the date when the person subscribed to your services? Also, what exactly happens when somebody hit the 1000 views?
