Our form was not working correctly

Noticed our form was not working correctly.  We reviewed the conditions and noticed some of the value fields were missing.  We had not changed anything yet this is the 2nd time this has happened.  Why?  Is this a glitch in the system.

Which value was missing? Value that is entered inside of condition? 

I inspected your form and I did not found any missing values, so I assume that you have re-added them. Did you maybe edited field options? If you edit the field option, value that was set for that option will be removed from condition.
