Form randomly deleted questions

Our direct deposit form suddenly had the banking info for routing and account numbers disappear off of the form and on all of the old submissions.  Those questions have been on that form since day it was created and we havent done any recent changes to it.  Any idea what happened and any way to get that missing info back from the previous submissions being its pretty vital info we need on file?
