Form integration with Box.com is currently broken, assistance needed

I have checked the form you provided and cannot find an integration with Box; however, I have integrated a testing form on my end and it seems to be working now. 

Could you please try integration your form with Box and test if submissions are sending? 

Please, also provide us more details about when the issue happens, if it never sends submissions to the Box folder or if it happens after some time.
