Form is not mobile responsive

    I added the Mobile Responsive widget but the form fields to not break to 2nd line when viewport is adjusted or on mobile devices with smaller viewports.

    I checked the Page URL you have provided and I can see that it is mobile responsive. Here's a screen animation of the form:

    Are you referring to the Header Image of the Form?

    If you're referring to a different issue, could you please provide a screenshot so we can check it further?
