that they send the ticked answers to an email.. so I can auto send them the URL of the site ie

*Your hair loss has started and is now speeding up.
*You've already lost much hair, and now showing male pattern baldness or you've lost from all over the head.. ladies.
*You've had a hair transplant but you're still losing hair from the rest of your head
*You've lost much hair from chemo, alopecea, hair lice, fungal infection, pharmaceuticals and/or stress.
Your email.. ie johnsmith4@gmail.com
NOTE to JotForm.. When they send email with ticked Qs.. my site URL is sent to them automatically and the email is preserved on file??

Creating a form with JotForm is easy as 1-2-3. You can just drag and drop the required fields to the form to create a form. Please refer to the guides below that can help you with creating forms.

When you add an email field to the form, an auto-responder will be added automatically to the form to the form submission data to the user. You may edit the auto-responder from Settings > Email > Auto-responder to add the URL of your website. Please refer to the guide below that can help you with adding an auto-responder to the form.
