In the past 2 weeks, we received a few duplicate submission which was first received back in June.
This never happened before.

Luckily, it does not happen often. It happened 3 times in the past 2 weeks. During the past few weeks, we have been receiving forms just fine as well.

So far we have not received reports about this same issue, it may be an isolated issue.
