I would like to do this, but would like to use the lightbox option. Is this possible?

I am not sure but i think it is only possible with hidden boxes in the system.

Yes, I was using the hidden box in the form.
But I want to display the form using a lightbox after clicking a link, rather than embedding the form directly on the page.
The embed code looks different w/ the lightbox option, so I'm not sure how to follow the instructions for finding out what URL the form is coming from.

You may want to check Adding-parameters-in-Lightbox-and-Feedback-forms article in JotFrom guide to understand how to pass value in a lightbox form.
This the lightbox code I used
windowTitle:'Clone of Clone of Contactformulier',

When using "g=feedback" instead of "g=feedback2" in this line:
the lightbox form pop-ups not in the middle of the screen, but near the bottom of the page.
When "g=feedback" everything is OK.
What is the problem?
