How can I create a table in a form?

Hello. 

I am wanting to create a table in a form that can be filled out by the employee. I have tried the survey tool but that doesn't suit my needs. 

I have attached an image of an excel table to give you an example of what I am trying to achieve. 

I have searched your forums and tried lots of the widgets but cannot seem to find the answer to this?

I was able to create a table from your image by using the input table field:

And it could like this:

It required some Custom CSS, you can check it in Custom CSS Field.

For the second one I used Configurable list widget:
