Photo is not displayed at my form

i build this form last year and it was good

but after couple of months my thank you page start to doing troubles

i build thank you page with some condition

it has the calculate button and when you press it you suppose to go to 1 of 4 pages 

the page look good but the pic i insert from my facebook account is not displayed 

like the like is broken , in my facebook the pic is ok and in the same album with the same name 

it is the second time this thing happens and i don't have any indication about it only when someone 

fill the form and tell me that it's not working

please see what you can do to help me restore the pic so they work again 

its hard work to put the pic back manually and design it to fit the the size and place

hope to ear from you soon

I noticed that you are using images which are coming from JotForm's cdn source. If you visit that link you will notice that the image is actually not available. As a result, on your thank you message, the image practically will not appear likewise.

I would suggest to get your image and place it somewhere other than facebook where it can host images and will have no problem being shared, such as your website and have a folder there where it will be your image hosting server.

As explained by my colleague, the image is not displayed since the URL used in the source code of the form is not available/accessible. Please try to upload the image on any image hosting sites like http://imgur.com/ or http://tinypic.com/ instead of facebook and add/use the link to the image to be displayed on your Thank You page.
