    500 Internal Server error

    Hi, I embedded the script tag for my new form into my php 5 page with it's own DIV tag. I'm having a hard time with the 500 Internal Server error.

    The website is using Java 1.7. Is this an issue?

    Hello kkelso,

    I'm sorry for the trouble caused to you. I did check your webpage where you have embedded your form and it seems to display 500 Internal server error message. 

    I believe there is some other problem with your webserver as it unlikely that form script code will display 500 internal server error. I would suggest you to please remove just the script embed code from your webpage and see if webpage is displayed normally. I would also suggest you to please try to embed your form using its iFrame embed code and see if that solves your problem. The following guide should help you how to grab your form's iFrame embed code:  https://www.jotform.com/help/148-Getting-the-form-iFrame-code
