Terms & Conditions Widget: Why widget is not included in emails or submissions reports?

    Hello,

     

    Here's a link to our form.  https://form.jotform.us/saltadministration/MedicalInfo

    This form is a field trip release form for students at our co-op.  One question has you check a user/agreement widget that is part of your standard widgets.  The problem is when I test out the form, this information is omitted in the notification email, response email and the attached pdf.  This is the box they check if they are allowing permission for their child to attend a field trip and it is critical that the pdf shows if they checked or did not check this box.  Any help you can provide would be great!

    I have been testing your form and I was able to replicate the same issue you are reporting, even on the submissions page the widget seems to be empty. I cloned it and was able to see the same issue as well. 

    I tried changing the fields name and deleting all the other fields on the form in order to find if there was a conflict with another field or widget; however, that does not seem to be the case. 

    I finally updated the widget from the wizard and that seemed to work, I would suggest you to kindly do the same on your end, please, open the widget's wizard and simply click on the update button:

    Thank you so much!  It worked.  I guess I should've realized that I needed to click on 'update widget'.  I love jotform and their customer support team!!  Thanks!
