Hello sarterberry,
Please note that form limits are applied to your account and not reset monthly. At any given point of time, you can only have 5 forms in your account.
I would suggest you to archive form which is currently not in use since the form limits is only related to active forms you have in your account. If you have reached the account's limit, you won't be able to activate these forms again.
