I'm trying to contact someone using their JotForm

    I filled it out, my name and email address but when I sent it, it's turned up in my inbox. When the form asks for name and email, should I be using theirs? The form is on their contact page so I assume they would need my details. It doesn't make a lot of sense. Will they receive the jotform too? I'm confused.

    Hi,

    Based on the link you sent, you'll fill out the form with your details. The email you received is an autoresponder email that confirms the submission. JotForm.com is an online form builder that allows anyone to create their own forms for personal or business use. For example, you have a small company with a website, and you wanted to have a contact form on it without the need of a programmer or any complex setup, JotForm allows you to create your own forms with drag and drop, and with ease. 

    In this case, I suggest contacting that company directly for your inquiry. I see that they provided their phone number and email address:

    "To book an appointment please call 0414 014 985 or email: info@uniqueupsychology.com.au"
