How to get conditions to show/hide properly?

If an applicant first answers Yes to "Are you a US citizen or permanent resident" and then selects either US citizen or US permanent resident, the correct field appears re documents required. If the answer is changed to No, the "Required Docs" fields remain visible. The conditions appear to be set correctly and should hide these two fields if the answer is No.

I assume you're referring to this:

That the fields revealed by 'Yes' remain if you switch to 'No'. 

Go to Settings-More Settings and scroll a bit down:

And select Clear on Being Hidden.
