My form has suddenly stopped redirecting to the page i need it to redirect to??

This sounds like it may be very frusterating and I'm sorry it has happened. In order to fix your problem I would suggest you go back into the related form and clear out your url redirection save it then re-enter it and save it again to see if that resolves the issue. It is possible that a glitch in the url may have occurred causing more problems for the coding. Lastly, if that does not work, it may seem disheartening but you may need to create the same fields on a new form and use that incase the original form glitched out which may be the case in things that bug out like that.

Changing redirect url didn't help.
Also noticed that the form submissions aren't being received.
If I go about making a new form, what is the likelihood of
this happening again?

Your form seems to work fine here. It redirects me to this page after submitting the form. Is this correct? Can you please try to clear your browsers cache and retest your form? Check my test here: http://idarktech.com/jotform/video/form_redirect.swf

I managed to fix the redirect by duplicating the form and them deleting the
Seemed to do the trick
