My form has a submit button for registration that will be billed indirectly (to a business) along with a separate submit button for those who will use a credit card (direct bill to the registrant). Both submit buttons worked prior to adding a Square integration. Now, my first submit button is not working. Is there a way to make the form work with both scenarios, or do I have to create separate forms for the two different types of registration?

I checked your form - http://www.jotform.us/form/71654046857161 - and now, it only has one Submit button. I went ahead and checked the form's history and 1 hour ago, there were still 2 buttons. Please see below.
Are you referring to these buttons? If so, I would suggest you revert to that specific version of your form to restore those buttons. Please see How to View Form Revision History
