I need help making limits on my form

I am offering sign up for a class, with 2 different dates. I have one form, they can select the date. I need to be able to disable the form after 5 from each date, not just 5 total, and I can't figure out how to do this. 

Please try our Gift Registry widget or Quantity Gift Registry widget. After that, set the Form Status to Disable on Submission Limit.
