    Hi there,

Although I got this to work, I realize now it caused some problems w/ the website. I use shopify for my website. On every page that I added the form, as explained above, I can no longer see the price above the add to cart button.

Here is an example of a page without the form: http://www.handstellstories.com/collections/home-decor/products/tree-of-life

Here is an example of a page with the form: http://www.handstellstories.com/collections/home-decor/products/bird-pillow-shams

    Any ideas?

This is a re-post of a comment on Capture the Page URL Where the Form is Embedded

I would suggest you to embed the iFrame code instead of the javascript one. I am not sure, but probably other existing js code in your page might be in conflict with the Jotform code

    Please give it a try.

Do not hesitate to look for more assistance if you need it

Thanks Jeanette, Although I am wanting the form to be available through a link, rather than be automatically visible on the page -- but at the same time, I want to be able to get the URL of the page that led the customer to the form, so I cannot put the form on its own page... Hmm.. any ideas about this?
