Deleting phone numbers

Hi,

If you do not have this problem, then I must be doing something wrong.  When I see the response form (before I click the edit), phone numbers are there.  As soon as I click 'edit', they disappear.  When I click cancel edit, they come back.

It didn't happen for the first two pages of our registration form, but on page three when I try to edit the response form so I can add separate text boxes for first and last names, this is where I'm having the problem.  Can you please check page three, the first name that is blank for first and last name?
