How can I remove a couple text fields with HR tags from my form?

Hello,

On some of our Forms, we have this <hr> that shows up randomly. I didn't create these forms, so I am having trouble locating how to remove them.

Can you please look at the link: https://form.jotform.com/40196370801147 - if you could 'no-go to order' and then 'next', you will be able to see what I am talking about.

While editing your form, scroll to page 5:

Click on the text fields that have the <hr> tags then click on the trash icon to remove the field.
