I was able to replicate the issue when I opened your web page. Here's a screenshot:

Please try to re-embed the form using the iFrame method. This will prevent script conflicts between the form and the web page. Here's a guide: https://www.jotform.com/help/148-Getting-the-form-iFrame-code.
