    Can I have a form which people could print out or get a copy as well as our office receiving the submission?

    This would be used for a dental office for patient registration. If they fill out this information (the end user,) can they also get e-mailed a copy of what they submitted?

    Hello Mark,

    If I understand your question correctly, you want to email a copy of the submission data to the user who submitted the form. Is that correct?

    Yes it is possible to achieve your requirement and here are the steps you need to follow:

    #1. Add a form auto responder email in form and configure it to be sent to the user's email address. The following guide should help you how to add form auto-responder email alert in form:  https://www.jotform.com/help/26-Setting-Up-an-Autoresponder-Email 

    #2. Enable "PDF Attachment" setting form under "Advanced" tab of auto-responder email. This will attach the submission PDF in email. Please check the screenshot below which should help you:
