We want to create a form to search on a input by user to a set database.
If some one puts his number in form
Then we what to check if the number exist in database or not.
And give response on it as if it exist message to appeared the number exist.

We do not have a built-in feature with that functionality but you can definitely use the form source code as the fundamental structure. Then modify the form source code and add your custom scripts that would perform the database checking.

Yes the widget will work for us, But can we modify it as we need it ?

The widget itself is not editable.
If you need to update data, just upload your updated spreadsheet to the widget.
