My form is a menu selection. It is long and i only want selections to appear when an email is received, not the whole form.

How do i make unselected items hide themselves?

The default email should have empty fields hidden by default.  Try deleting your current notification and adding a new one:

That should allow for the email to have empty fields hidden once again.
