We recently started noticing that our employees are not receiving a conformation email after passing a quiz. How do we get this to stop happening? I checked all of the settings and they seem to be right.

I have checked your form and noticed that your email is conditionally sent, please do note that the email will not be sent if the conditions are not met.
I also noticed that you have not specified the email address where the quiz results will be sent to:
I would suggest you add the recipient in the "Email results to" section, this way he/she receives the quiz results as well. Also, do note that the emails sent through the quiz app cannot be conditionally sent.
Please, test it again and let us know how it goes.
