Why submit button does not appeat on my form?

I have created my form that I would like to attach to the end of a class event online in facebook tonight.  When I add the link to my class and click on link it does take me to the form however there is not a SUBMIT button for people to submit the form after they have filled it out.

Also, how will I see the data from them filing it out
