I am not receiving my emails with my form

I have submitted this issue a couple of times and never have heard anything back.  We have had a form come to Jotform but the form never reached our email.  PLEASE CALL ME as I cannot find a number to call you.  We are a paying customer and don't feel this should be an issue.

What is the form in which you are not longer receiving email notifications? Please check your spam folder, if you find the notifications there, make sure to mark them as NOT SPAM.
