How do I receive payments?

I've created a Payment Authorization Form however I'm not sure how the money is transferred to my company's account when completed. 

Do I need to fill out an ACH form? How does that work?

Payments can only be accepted through one of our integrated payment processors.

It looks like you have added an Authorize.net payment field to the form.  This field would require you to have an Authorize.net account in order to process the payments, adding a PayPal field to the form would require the use of a PayPal account to accept payments, same for all other payment processors.
