My form is not publishing

    Hi,

     

    My form is not loading properly. It says it is disabled. However, I checked my setting and its enabled. Please help.

    Your account was autosuspended for collecting usernames. Users are not allowed to collect usernames/passwords/credit card details. That goes against our terms of use. 

    The good news is that we should be able to reactivate your account once the offending fields are removed.
