I have noticed that your emails were successfully sent to the submitter's email address. In case you are experiencing issues with your form, please try using an SMTP sender. This guide will help you doing it: https://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form

I tried to try using an SMTP sender following the guide you sent, but I couldn't get past the step where I need to select "add a new email address" it wouldn't let me make that option.
Your assistance is appreciated. In the meantime, we have disabled the form so we don't have issues with our registrants.
