I'd like to have a custom domain setup for the form I built, is there a way to setup the weblink to be survey.pacificsuntech.com

Yes its possible. You would have to lease a domain name and point it to your form:

No, you must purchase a lease from a hosting provider (like godaddy.com). They will all have an admin panel where you can specify your DNS settings. You will need to specify a CNAME DNS entry. This tells anyone who accesses your domain to be sent to the jotform domain.
