    Yes its possible. You would have to lease a domain name and point it to your form:

    No, you must purchase a lease from a hosting provider (like They will all have an admin panel where you can specify your DNS settings. You will need to specify a CNAME DNS entry. This tells anyone who accesses your domain to be sent to the jotform domain.
