Also, does this an automatic yearly subscription or does it need to be resubmitted every year? Thanks.

You are on the FREE plan. Also you are overlimit. So your forms are currently not accessible.
On the FREE plan there is no yearly subscription or costs invloved.
However, you are limited to 100 submissions per month. And 1,000 form views per month. If you go over your limits your forms get disabled.
When this happens you have two solutions:-
#1. Wait until the 1st day of next month. JotForm resets its counters on the first day of each month.
