How can I create a form with a download

I want to create a form for people to add in their Name and email address.  Once they provide that information and submit the form, I want either 1-- the Thank You page has a link to a PDF of the download OR 2- they get an email with the PDF download.  How can I do that?

A PDF link can be added to both the thank you page and email autoresponders.  While editing the thank you page, you can insert a PDF link from the "Form Fields" tab:

To include a PDF in the autoresponder email, you can enable the PDF attachment under the "Advanced Settings" tab:
