    Hello. please tell me why is my form currently unavailable ? How to fix this ?

    I see that your form has been disabled. This either was manually disabled, or you have set a scheduled disable date of the form. In any case, please check out this guide to know how to enable this again: https://www.jotform.com/help/180-How-to-Enable-Disable-a-Form
