I checked your account and I can see that it has four forms active. Upon checking the Archive folder, I see another form that it is in active status. Could you please try to disable it in the Form Settings page? To do that, please follow this guide:
1. Log in to your account.
2. Go to the Archive Folder.
3. Search for the Form ID: 50607166469158
4. Select the form and click the Edit button.
5. Go to the Form Settings page and disable the Form Status.
