I checked your account and I can see that it has four forms active. Upon checking the Archive folder, I see another form that it is in active status. Could you please try to disable it in the Form Settings page? To do that, please follow this guide:
1. Log in to your account.
2. Go to the Archive Folder.3. Search for the Form ID: 50607166469158
4. Select the form and click the Edit button.
5. Go to the Form Settings page and disable the Form Status.I hope that helps. If the issue persists, please let us know.
