Is it possible to remove the JotForm branding from my form?

We have a paid account but I'm still seeing JotForm branding on the form and on the confirmation page someone sees after they submit the form. Is it possible to remove that branding?

I have cleared your form caches then proceeded with testing the form you kindly pointed us to, and I do not see the JotForm branding in it.
