The recipient e-mails field only allows me to add e-mail address instead of designating a field to pull this information from? In order to set a field on the form as the recipient email address, it is required to use an auto-responder instead of a notification. Unfortunately, for notifications, it is not allowed to use the field names as the Recipient email. Please try replacing the notification with an autoresponder so that it should be working fine.
