Embedded form not displaying fully on iphone

Hi,

I have a form embedded on an unbounce page.

When viewing this unbounce page with an iphone and clicking on the button to open the jotform that is embedded in a lightbox, the form is displaying only partially. 

I've used iframe code and included mobile responsive widget in the form.

Can you help me understand why the form is not displaying correctly on an iphone?

 I checked the page URL that you have provided using iPhone 6 and I was not able to replicate the issue. I opened your form using the Google Chrome built-in mobile display viewer. Please see the screen animation below:

If the issue persists, please try embedding the form using the iFrame Method but use the code shown in the screenshot below:
