If I would upgrade to a Premium account, would there just be one login/password, or would different individuals be able to have unique credentials?
Also, if we would upgrade to Premium, would the JotForm branding at the bottom of my sample form disappear when the consumer is filling out the form?

If you upgrade to a Bronze account (Premium is no longer available) you can add up to 3 other accounts as sub-users and give them access to forms without having to allow them to log in to you account:
The JotForm branding will be removed from forms automatically with a subscription of Bronze of higher.
