Conditional answers set

How can I define a set of a 5 questions/answers, where each answer can be chosen only once?

If you want to allow your users to select only one answer you should use the Single Choice (Radio button) field.

Do you maybe want to create 5 fields that will have same the options/answers and you want to prevent your users to select the same answer more than once? 

Unfortunately, it is not possible to disable a field option if the same option was previously selected on same another field with conditional logic. 

In this case, I would suggest you to use a Visual Multi Select widget. With this widget, your users will be able to select the order of the options/answers.
