    Hi,

How can I define a set of questions/ansers where each closed answer (let's say a score 1-5) can be choosen only once?

    Thanks!

I made a quick demo for you:

#1. Add a input table for each question you want to ask. Configure it so that it only has one row. And and as many columns as you like. I did one row with five columns.

#2. Open the properties for your input table (blue gear icon). And on the Options tab specify the score values for each column seperated with a comma. Like this:

#3. Add a form calculation widget. Just search on the widgets tab (left hand side). And drag it onto your form.

#4. Then click on the wizard (magic wand icon). And setup your calculation like this:

Click the ADD FIELD button for each question. And then put plus signs between them. Next divide the result by how many answers there is (which is currently 8) and multiply the result of that by 100 to get a percentage.

Thanks for the super detailed reply, but i'm not quite sure what you did on the last stage.

In addition, I'm afraid I didn't explain myself clearly.

For example I wish to provide 5 rows (service, kindness, etc) and for each one user should rate and can't give the same rate twice. let's say in case service got the score 5 (or 'extremely pleased'), other rows can get only other values.

I would suggest you to check the following guide in order to know more details about calculations: https://www.jotform.com/help/259-How-to-Perform-Form-Calculation-Using-a-Widget

Regarding to your second question, as I can understand, you need to disable the options in the same column once one of them has been selected.

As far as I know there is not a way to achieve it; however, you may let us know if this is what you're looking for, we will be glad to open a feature request for this.

Thanks to you all for the awsome support!

I have narrowed my options to Orerrable List + Visual Multi Select Widgets which handle my need.

    Will let my client choose.

    Thanks again!!
