Why do I get a "Form disabled" message after submitting my form?

    I just created a new form with a file upload utility built-in. My attached file, a PDF, was only 2MB (max is 20MB).  All file extensions are accepted. After submitting the form, I was taken to a page that simply said "Form disabled" with no other explanation. I also have Google Sheets activated and a sheet with the form contents was created in my GDrive but the form contents submitted did not write to that Google Sheet. I have no idea why the form is disabled or how to fix this. Please help, if you have ideas. Thank you.

    I reviewed your form but I could not reproduce the issue at this time.

    You can test submit your form again. Let us know if issue persist.
