How to create happy/sad smile survey questionnaire?

    the idea would be a simple click on happy or sad faces to collect satisfaction score. I tried multiple response template, could get a yes/no reponse in written form, but couldn't see how to replace checkbox with happy/sad icons. Any idea how best to do this?

    I used happy/sad face by searching the Google, and just copied the URL.
